Attention students with_missing_or_NA_external_result

Students whose external results are being displayed as 'NA' or are missing from the results even though they have given exams (theory/practical) are advised to fill out this form.

Please note that this form has a separate section for each subject. Each section has a heading "Problem n" where n is a natural number. DO NOT UPLOAD ALL SUBJECT DETAILS IN ONLY ONE SECTION. For example, if you have a problem in two subjects, say Chemistry lab and Physics lab, then first upload details of just Physics lab, then answer 'Y' to the question "Any other issue" and you will automatically be redirected to the next section where you can upload details of Chemistry Lab.

Form submissions where subjects are not segregated will be summarily dismissed.
